On July 21, 2026, Nature Medicine published two small trials from UC San Diego and Rady Children's Institute for Genomic Medicine. The patients were two boys with SCN2A-related developmental epileptic encephalopathy, a severe disorder in which one misspelled gene packs a childhood with seizures and stalls the ordinary work of learning to speak and move.
The older boy had never walked on his own. Within two years his seizures fell by ninety percent, and at fifteen he stood up and crossed a room. When the drug wore thin a few weeks before his next dose, the walking faltered; the team gave it more often, and his legs came back.
The drug is not built for epilepsy. It is built for his epilepsy: a strand of synthetic DNA tuned to silence the one faulty copy of the gene and leave the healthy copy alone. "You can't get more personalized than that," said Olivia Kim-McManus, who led the work. Each boy is his own trial and his own control, a population of one.
A shepherd has a hundred sheep and loses one. He leaves the ninety-nine in the open country and goes after the single missing one until he finds it. The divine gaze does not blur into a crowd. It lands on the one, the way this therapy was bent around a single boy.
The economics might not always add up. You cannot mass-produce a shepherd who leaves the flock, and you cannot mass-produce a drug built for one child.
None of it is finished, the younger boy gained less and two patients is not a cure, and still the trials show what no population average ever could: attention this fine is possible, and it can put a boy on his feet.
