What this verse means
The prophet Jeremiah says the human heart is so tangled and self-deceiving that no person can fully understand it, and the very next line adds that God alone sees it clearly.
Jeremiah was a prophet in Jerusalem in the years before the city fell to Babylon, warning people who were sure their own plans and alliances would keep them safe. Through this stretch of the book he keeps setting two kinds of trust against each other: leaning on human strength, or leaning on God. This verse is his blunt verdict on the first one. The heart here means the inner self, the place where a person's real motives live, and his claim is that it can fool even its own owner. We do not always know why we do what we do.
The question at the end, “who can know it?”, does not stay open. In the very next verse God answers it: he searches the heart himself and gives each person according to what they have actually done. Our motives can stay hidden even from us, and they are still fully known to the God who weighs a life by what it truly intends.
